Renewing my Disability!

I have to send the form to my psychiatrist every year to renew it. Those on it, do you have to be re-evaluated by your psychiatrist to keep on having it? I have to renew it every 12 months myself anyways. Do you have to, too personally?

Here you can be awarded it for between 1-10 years or even life depending on what your disability is. You also have to go for an assessment then the assessment doctor etc will send a report to the disability office. then after the award is coming to an end they contact you to renew it.
